Sugar Paws Group Class Intake Form

Please fill out the information below, and then submit this form. Once that’s complete, please submit payment either through the website or at Sugar House Veterinary Wellness Center to reserve your spot in the class. Reservations cannot be guaranteed until payment is received.

Once both tasks are complete, you’ll get a receipt immediately and a confirmation/reminder email from The Canine Life coach LLC within one week of the class start date.

• Liability Waiver & Policies: The Canine Life Coach LLC. will endeavor to create as safe an environment as possible for the training of my dog and will endeavor to offer only sound, safe, and responsible training and training instructions. However, I recognize that Canine Life Coach LLC is not responsible for any unintentional errors, omissions, or incorrect assertions. I understand that the recommendation of any other product or service is not a guarantee of my satisfaction with that product or service. Further, I am and will remain responsible for the actions of my dog at all times. I hereby agree to abide by the rules and policies of The Canine Life Coach  LLC training classes as set forth in this contract. I understand that attendance of dog training classes is not without risk to myself, members of my family, guests who may attend, or to my dog. In consideration of, and as inducement to the acceptance of my application for training membership in this training class, I hereby agree to indemnify and hold harmless The Canine Life Coach LLC, its officers, directors, instructors, agents, employees and/or representatives of any and all claims, or claims by any member of my family, or accompanying guests of mine of injury, expense, costs or damages to  myself, my dog or any handler sponsored by me both in class and out of class. In addition, I agree that I will defend and indemnify The Canine Life LLC for any injury, expense, costs or damages to any dog handlers or dogs, whether sponsored by me or not, or to third parties arising out of my own actions or the actions of my dog. I have read the above-stated provisions, and agree to accept those responsibilities.

• Photo & Video Release: I grant permission to The Canine Life Coach  LLC, the irrevocable right to use my photograph(s), likeness and voice, including via video, and those of my dog(s) and of any minor children of mine who may have been photographed or recorded, for the following: Instructional purposes solely with me, members of my family or household, or other persons who regularly handle my dog, such as other dog care professionals hired by me, or other service professionals hired by me (such as nannies, housekeepers, and the like). Instructional purposes for other clients or students of The Canine Life Coach  LLC, including one-on-one sessions, group classes, web seminars, in-person presentations, etc. Marketing purposes for The Canine Life Coach  LLC, such as but not limited to use on The Canine Life Coach's website, social media channels, webinars, in-person presentations, email newsletters, and print materials.

• I hereby release The Canine Life Coach LLC and its legal representatives and assigns from all claims and liability relating to said photograph(s), video(s), likeness and voice, indicated by my initials below.

• I accept the terms of the Canine Life Coach Refund Policy.*

• I understand that once a class starts, no refunds will be issued for any registered participants. If you miss a class you will receive the homework for the week and accompanying video of the skills covered in the class. There are no make-up classes. You will be caught up and with the rest of the class. No refunds will be given.
